Abraxas Youth & Family Services is hiring a Case Manager for our Lehigh Valley Community Based Program.

As a Case Manager, you will be responsible for a caseload of adjudicated delinquent or dependent youth and/or their families who have been referred to a community-based programming.
You will play an important role in assisting our client (and their family) in developing a multi-dimensional approach that assists clients/families in creating a solid foundation from which to build positive, sustainable change for the future.
Salary: $21.
42 per hour Job Type: Full-time Shift: Flexible Schedule Case Manager Hiring Requirements: High school diploma and minimum of three years of related experience in the juvenile field; OR Bachelor’s degree in related field preferred and minimum of one year of related experience in the juvenile field; OR Master’s degree in related field and minimum of six months related experience in the juvenile field.
At least twenty-one (21) years of age Valid Driver's License.
Physical exam that includes TB and drug testing.
Criminal clearances (State Police, FBI and State Child Abuse Clearances) Satisfactory completion of background screening and applicable pre-employment checks; including but not limited to employment and/or personal reference and driving records.
Ability to work with computers and the necessary software typically used by the department.

In Summary: The Case Manager is responsible for a caseload of adjudicated delinquent or dependent youth who have been court-ordered to residential placement or community-based programming.
The position is responsible for successfully assisting the client (and the family) in transitioning from residential placement back to the community or working with youth directly referred from the community.
Salary: $22.

Develop and maintain relationships with community service sites.
Develop and maintain relationships with assigned Juvenile Probation Department.
Orient the client to program norms, rules, and behavioral standards.
Collect the YLS assessment from the Juvenile Probation Department and implement it into the service plan and the service delivery.
Build and maintain a positive relationship with the youth and their family through being caring, consistent and competent.
Support clients and their families in completing any court ordered conditions including restitution, community service, or specific counseling needs and assessment.
Supervise client activities and enters at least one note for each client into the Echo Advantage System bi-weekly.
Communicate on a bi-weekly basis with the client’s Probation Officer.
Maintain a receipt book for all restitution payments.
Makes timely payments of all restitution to the Program Specialist at WorkBridge or the Clerk's Office at Allegheny County Juvenile Court.
Engage with clients prior to release from residential program to complete intake/interview process as needed.
