Job Summary:
You will assist clients with needed resources and navigating multiple social services and health systems, link clients to appropriate care and ensure clients achieve their wellness and recovery goals.

What You Will Do:
Facilitate and coordinate psycho-educational services for consumers and their family members relating to wellness, health promotion, prevention, skill-building, and recovery topics
Offer on-site and outreach services (including transport of consumers)
Fulfill case management services, including coordinating or directly facilitating linkages to services, service coordination, and treatment enrollment assistance.
Ensure consumers have community supports in place that fulfill a positive corroborative role
Communicate observations and information about the consumer in the consumer's service record and to team members
Participate in supportive counseling, crisis intervention, quality assurance activities, and cross-functional action teams
Participate in the assessment and evaluation of consumers at intake/admissions, during ongoing recovery, service delivery, and discharge
Collaborate with team and treatment providers to establish comprehensive bio-psycho-social assessments and care plans
Contribute to the formulation of consumer-centered care plans in collaboration with team members, containing relevant, measurable goals and realistic time frames for achievement; perform progress reviews and evaluate the effectiveness of goals.
Work with administrative staff to ensure that all services are documented based on billing and regulatory standards.
What We Need to See:
Minimum of High School or GED equivalent, BA, preferred
1-2 years relevant work experience.
CADC (or LCADC)
A valid driver's license and must be insurable by the agency's automobile insurance
